Estado de la prescripción dietética en un hospital clínico quirúrgico terciario.

RESUMEN

Se pretende evaluar si el acto de la prescripción dietética hospitalaria se realiza siguiendo alguna pauta científica. Para ello, se auditaron las historias clínicas de 460 pacientes seleccionados aleatoriamente de entre los ingresados entre Agosto y Diciembre del 2009 en el Hospital Clínico quirúrgico “Hermanos Ameijeiras” (La Habana, Cuba) para recuperar la figura dietética corrientemente prescrita. Se recuperaron 445 (96.7%) de las figuras dietéticas esperadas. La figura dietética recuperada fue examinada para comprobar si aportaba información sobre la composición nutrimental (energía incluida) de la prescripción dietética, y la textura y consistencia de los alimentos a servir. La figura dietética también fue analizada en el contexto de la enfermedad de base del enfermo, a fin de saber si se restringía el ingreso de algún nutriente para la paliación de los síntomas y la progresión de la misma. La “dieta libre”, la “dieta hiposódica”, la “dieta de diabético”, la “dieta blanda”, y la “dieta hipoalergénica” fueron las figuras dietéticas más prescritas, y explicaron el 52.3% de las figuras recuperadas. Solo el 11.9% de las figuras recuperadas informó sobre la energía a aportar, mientras que el 8.3% lo hizo sobre el contenido de proteínas de la dieta hospitalaria. El 66.7% de las figuras describió la textura y consistencia de los alimentos a servir. En 272 instancias la figura dietética informó sobre restricciones de nutrientes en la dieta hospitalaria. Se concluye que la prescripción dietética hospitalaria no sigue ninguna pauta científica. ABSTRACT

It is intended to assess if the act of the hospital dietetic prescription is conducted following any scientific guideline at all. For that purpose, clinical charts of 460 patients randomly selected from those admitted to the Clinical Surgical “Hermanos Ameijeiras” Hospital (Havana City, Cuba) between August and December of 2009 were audited in order to retrieve the currently prescribed dietetic figure. Four-hundred forty-five (96.7%) of the expected dietetic figures were recovered. Retrieved dietetic figure was examined in order to see if it provided information about the nutrient composition (energy included) of the dietetic prescription, and the texture/consistency of foods to be served. The dietetic figure was also analyzed within the context of the patient’s primary disease in order to know if intake of a particular nutrient was restricted for palliating the disease’s symptoms and progression. “Free diet”, “Hyposodic diet”, “Diabetic diet”, “Soft diet”, and “Hypoalergenic diet” were the most prescribed dietetic figures, and explained 52.3% of the retrieved figures. Only 11.9% of the retrieved figures informed about the energy to provide, whereas 8.3% of them did it regarding the protein content of hospital diet. Sixty-six point seven percent of the figures described the texture and consistency of the foods to be served. In 272 instances dietetic figure informed about nutrient restrictions in the hospital diet. It is concluded that hospital dietetic prescription is not conducted according to any scientific guideline at all.
